When to Submit Medical Updates
Please notify us of any of the following developments in your case:
- ✓ Visits to NEW doctors or specialists you haven't told us about before
- ✓ New diagnoses or significant changes in your medical condition
- ✓ Hospitalizations or emergency room visits
- ✓ Changes in medications or treatment plans
- ✓ New test results (MRI, CT scan, X-rays, lab work)
- ✓ Starting physical therapy, mental health counseling, or other new treatments
- ✓ Surgical procedures or scheduled surgeries
📋 Note: Routine Follow-Ups
You do NOT need to report routine visits to doctors we already know about (like your regular primary care provider) unless something significant happens during the visit (new diagnosis, medication change, referral to specialist, etc.).

How quickly will someone review my update?
We review all medical updates within 1-2 business days. For urgent matters, please call our office directly.
Do I need to submit every doctor visit?
You only need to report visits to NEW doctors we don't know about yet, or when something significant happens at a visit (new diagnosis, medication change, test results, etc.). You do NOT need to report routine follow-ups with doctors we already know about unless there's something new to share.
Should I still mail or fax medical records?
You can upload records here if you have them in digital format. For paper records, you may still mail or fax them to us at (606) 519-7100.
What if I forgot to include something?
You can submit this form multiple times. Just submit another update with the additional information.
Will this delay my case?
No! Keeping us informed of medical updates actually strengthens your case and helps us advocate more effectively for you.
